Sean “Diddy” Combs was taken by ambulance from the federal prison where he is serving his sentence early Saturday after reportedly suffering a head injury, according to TMZ. The rapper was returned to the facility within hours, and the injury was described as non-life-threatening.

The ambulance reportedly left Federal Correctional Institution Fort Dix in New Jersey around 1 a.m. ET. Combs was back at the prison before 7 a.m., with sources telling TMZ that he was conscious and speaking when he returned. It remains unclear how he sustained the injury or which medical facility treated him.

A representative for Combs told Page Six that his team was attempting to verify the circumstances surrounding the reports.

“We are aware of the reports concerning Sean Combs and are working to confirm the circumstances,” the representative said. “Sean’s health and well-being are our immediate concern, and we appreciate everyone who has reached out.”

The medical incident comes as Combs faces renewed scrutiny over his treatment behind bars. Recent reports have alleged that the music executive received unusual privileges at Fort Dix, including access to special meals, massages and other accommodations. Reports also alleged that he possessed a contraband cellphone that he used to view explicit material sent by a former girlfriend.

Prison officials reportedly imposed disciplinary penalties after allegations involving the cellphone and other conduct. Combs lost 41 days of good-conduct credit, pushing his projected release date from January 2028 to March 2028, according to reports. He was also reportedly returned to solitary confinement after details of his alleged prison privileges became public.

The Bureau of Prisons has previously declined to comment on specific claims about Combs’ living conditions, citing inmate privacy, safety and security.

Combs is serving a 50-month sentence following his July 2025 conviction on two counts of transportation to engage in prostitution. He was acquitted of the more serious sex-trafficking and racketeering charges brought against him.

His trial included testimony about so-called “freak-off” sexual encounters involving his former girlfriend Cassie Ventura and other women. Prosecutors alleged that Combs arranged for male escorts to participate in encounters that sometimes lasted for days.

The circumstances of Saturday’s reported head injury have not been publicly established, and no further details about his treatment have been confirmed.
